Service Coordinator

A health services organization in New York City is currently seeking a hardworking and dedicated professional to join their staff in Queens as a Service Coordinator.
In this role, the Service Coordinator will be responsible for assisting the applicants and participants in obtaining and coordinating the services that are necessary to remain in or return to their home.
About the Opportunity Monday to Friday (1 to 2 days in the field; 3 days office) 8am to 4pm or 9am to 5pm 10 to 15 Patient Elderly and/or disabled The Service Coordinator Complete Individualized Service Plans and Plans of Protective Oversight (PP0) and meet with participants in order to gain an in-depth understanding of their goals Act as the participants advocate, provide unbiased assistance to the participant with the selection of providers, and assist the participant in locating a place to live in the community Coordinate and schedule waiver and non-waiver services Maintain consistent and open communication with the RRDS and waiver participants Assure the timely implementation of the approved service plan Conduct face-to-face meetings with the waiver participant at least monthly to monitor health and welfare needs Maintain case records consistent with all contacts and necessary documentation Maintain a tracking system that monitors all areas of compliance Perform other duties, as needed 2
years of experience providing service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors Bachelors and/or Masters Degree in Psychology, Social Work, and/or a related field Knowledge of waiver services, as well as current knowledge of informal supports within the participants communities Experience working with other service providers and Medicaid benefits Solid assessment and documentation skills Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized Desired Bilingual (English and Spanish) Recommended Skills Attention To Detail Communication Coordinating Hardworking And Dedicated Interpersonal Skills Medicaid Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
