Fee for Service LPN (Licensed Practical Nurse)
MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury or surgery.
With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.
Multple positions within the following regions:
Bronx, Manhattan, Queens and Brooklyn.
Reporting to the Coordinator of Care you will be responsible for seeing patients in their home for revisits based on a skilled need, implement the plan of care and evaluate the treatment.
You will document the visit electronically and completing documentation within 24-48 hours.
Daily visit rate:
$48Weekend / Holiday visit rate:
$55nursing, LPN, homecare, home care licensed practical nurseNY StateLPNLicense requiredMinimum of one year Med/surg experience in a hospital or Longterm Care facility required.
Must have a NY State Drivers License and a car.
Good computer skills required.
