Medical Coordinator/Direct Support Professional

Queens, NY Queens, NY Full-time Full-time $17 - $20 an hour $17 - $20 an hour Queens social services agency seeks a highly motivated and reliable person to work as a Medical Coordinator/Direct Support Professional in the IRA located in Queens.
DUTIES AND RESPONSIBILITIES OF THE JOB:
Medical Coordinator duties (not an inclusive list) 1.
Maintain scheduling of medical appointments/medications records/and ensure RN/MD treatment orders are implemented for all individuals.
2.
Prepare clinic visit consult record generated for each appointment.
3.
Review all consults immediately upon return to facility as per agency's guidelines.
4.
Process medication/treatment order(s) in house and with pharmacy.
5.
Ensure all follow up is completed for appointments attended by additional staff.

Daily review of MAR/Medication inventory to ensure adequate supply is on hand:

Prepare new monthly MAR.
8.
Liaison between RN and medical facility/office to ensure appropriate document- ation/ appointment/orders are received as needed.
9.
Meet weekly with RN and Assistant Manager/Shift Supervisor to review past week appointments/activities and review all outstanding referrals/reports/orders 10.
Attend Clinical Team meetings/ISP meetings/Annual and Semi-Annual meetings.
11.
Organize consent for treatment process 12.
Additional duties as required.
Direct Support Professional duties (not an inclusive list) 1.
Provide direct care services to the individuals who live in the IRA, including, but not limited to, training and assistance in ADL skills, independent living skills, community inclusion skills, productivity skills and recreational activities, with an emphasis on the individual desires and needs of each individual.
2.
Implement individual goals as scheduled and complete the data sheets accurately and fully.
3.
Administer medications as needed and document accurately on the Medication Administration Record sheets (upon completion of an approved AMAP course).
4.
Provide crisis intervention as needed (upon completion of an approved SCIP course).
5.
Provide CPR and First Aid as needed (upon completion of an approved First Aid and CPR course).
6.
Assist in maintaining a safe and clean physical plant.
7.
Identify, report, document and provide individual protection in the event of an occurrence, reportable or serious reportable incident or allegation of individual abuse.
8.

Uphold and protect the rights of all individuals.
9.
Remain awake and alert on all shifts, including overnight shifts.
10.
Perform other duties as assigned.
EDUCATION:
H.
S.
Diploma/GED required or Associate Degree is preferred EXPERIENCE 3-5 years' experience in Human Services or OPWDD related field.
Excellent time management skills required Excellent communication and listening skills REQUIREMENTS NYS Driver's license mandatory.
Must have a clean license.
